Registration
You may download and print out any of the camp registration/immunization/medical release forms using Adobe (PDF file format) by clicking on the links above. (If you to not have Adobe Reader or if you have any trouble reading the form, please click here and follow the on-screen instructions to download the latest version of free Adobe Reader software).
Be sure to check that your son/daughter’s immunizations are up-to-date. The form above (or it’s equivalent!) needs to be filled out and signed by a physician.
After printing and filling out a registration form for each child you wish to register, please mail it along with a $50 check (made out to All-Court Tennis) to:

Please note: Even though Pingree has eight new courts, there is a limited number of campers per session. Some camp sessions may fill up. We will make every effort to accommodate sibling groups and car-pool groups, but please indicate any groupings on registration to help facilitate this—and to claim the discount! Call if you have any questions.
After Registering:
We will confirm receipt of your registration by phone/email. If you are new to All-Court, please feel free to call (978-465-6564) any evening to ask any questions, or just say “hello.” We will telephone all campers one week prior to camp to follow-up on the medical forms and immunization records required by Massachusetts state law. A certificate of “Proof of Physical” in the last 24 months must also be provided before the first day of camp. For everyone’s safety, the local Board of Health is very strict about these rules and I am inspected each week prior to camp to be sure everything is in order.
